What to include
Patient demographics, your contact information, reason for referral, and urgency level. Attach any relevant records, X-rays, or scans — PDF, JPEG, or PNG, up to 10MB per file.
What happens next
You’ll receive an acknowledgment that we’ve got the case. We coordinate scheduling directly with your patient, keep you informed through the surgical episode, and return them to you for ongoing care — the referral relationship stays yours.
